- Interprofessional collaboration and care integration across services are essential for ensuring a safe transition from hospital to community-based services[1]
- This requires an integrated multidisciplinary approach to care provision across tertiary, secondary and primary care services
- It needs:
– meaningful interdisciplinary collaboration and effective communication, especially for vulnerable patient groups
-strong organisational leadership and infrastructure investment
-Individual, professional and organisational relationships built over time to promote effective interprofessional collaboration
However, universal communication mechanisms between hospital and community-based services need to be improved which is why the integrative care committee is such a vital point of advocacy in our region

A network of health care professionals in SESLHD and CESPHN.

To improve communication between general practices & St George Hospital, & to streamline the navigation of services & referral pathways.

We collaborate with stakeholders to create changes that drive improvements in care.
