Health Pathways

An online clinical decision support portal helping GPs assess conditions and navigate local referral pathways β€” developed collaboratively with CESPHN and local Local Health Districts.

πŸ”
Representative Position β€” Currently Vacant

The Division is currently seeking a GP to represent our community on the HealthPathways Committee. This is an opportunity to contribute to the development of clinical pathways used by GPs across the region. If you're interested, please get in touch.

What is HealthPathways?

HealthPathways is a web-based clinical decision support tool used by GPs and other health professionals across the CESPHN region. It provides up-to-date, locally relevant guidance on how to assess patients and determine the most appropriate pathway β€” whether that means managing the patient in general practice, referring to a community service, or escalating to hospital care.

Each pathway is co-developed by GPs, specialists, and allied health professionals, with input from the local LHD and PHN. They reflect the specific services, thresholds, and referral criteria that apply in our region β€” making them more useful and more reliable than generic national guidelines.

The Division participates in the HealthPathways governance and content development process to ensure that GP input shapes how pathways are written and maintained. GPs who use HealthPathways regularly can also provide feedback directly through the portal.

How GPs use HealthPathways

HealthPathways is designed to be used during or between consultations as a quick, reliable reference for clinical decisions and local referral processes.

1

Search a condition or symptom

Type a condition (e.g. "chest pain", "depression", "knee pain") and HealthPathways returns a pathway with assessment guidance and local management options.

2

Review the local pathway

Each pathway shows recommended investigations, what to manage in GP, when to refer, and which local service to use β€” with direct links to referral forms or contact details.

3

Follow the referral guidance

Each pathway specifies whether a condition should go to an outpatient clinic, urgent care, or ED β€” and what information to include in the referral. No more guessing referral thresholds.

4

Provide feedback to improve pathways

Found a pathway that's out of date or doesn't match your experience? You can submit feedback directly through the portal, or raise it with the Division to escalate through the HealthPathways Committee.

Become the Division's HealthPathways Representative

We're currently seeking a GP from the St George community to represent the Division on the HealthPathways Committee. This role involves attending committee meetings, reviewing proposed pathway changes from a GP perspective, and bringing GP feedback from the community to the development process.

It's a practical, clinical role β€” ideal for a GP who enjoys reading and contributing to clinical guidance and who wants to ensure that HealthPathways reflects the realities of GP practice in our region.
